enhance: Avoid initializing casbin enforcer for each request (#29117)
See also #29113

This patch:
- Replace plain Enforcer with `casbin.SyncedEnforcer`
- Add implementation of persist.Adapter with `MetaCacheCasbinAdapter`
- Invoke enforcer.LoadPolicy when policy updated
